APPOINTMENT CHECKLIST

Bringing the following items with you to your appointment will ensure that the doctor has everything they need to give you a thorough examination.  

We ask that you bring the following items with you to your appointment:

Current Glasses and Contact Lenses

Please bring with you any glasses and contact lenses that you are currently using, including sunglasses and computer glasses.

Previous Written Prescriptions

If your last examination was not with us, please bring a written copy of any prescriptions that you were given at your last exam. Contact lens wearers, if you are unable to find a written contact lens prescription, the contact lens boxes, blister packs, or previous order history will suffice.

Below you will find a link to our Patient Registration Form. Please fill out the form in its entirety and bring it with you to your appointment. You will need access to a printer.

A Mask

For your safety and ours, a mask is required during your time in our office. This is one of the changes we have made due to the COVID-19 pandemic.
