COVID-19 Information


You might notice some changes when you come in for your next appointment...
In response to the COVID-19 epidemic, we have made several changes to our office procedures. We are following recommendations from both the American Optometric Association and CDC for the safety of our patients, staff, and doctors. 

Masks Required for your Safety

For your safety and ours, masks are required during your eye examination. Please wear one to your appointment. 

While masks are currently not required inside the Costco Warehouse, California Department of Public Health states that masks are still required in health facilities. Therefore, they are required inside our office. We thank you for your cooperation.  

Screening Questions

When scheduling and/or confirming your appointment, we may ask you a series of screening questions related to COVID-19, such as: 
  • Are you experiencing any symptoms of COVID, such as fever, cough, difficulty breathing?
  • Are you currently ill or have you been exposed to someone who is ill?
If the answer to any of these questions is "yes", we will direct you to seek care with your medical provider and we will reschedule your appointment. 

Patient Registration

If you have access to a printer, we ask that you please bring your completed Patient Registration Form with you to your appointment. 

The form is an editable PDF document, so you are welcome to type your information into the document using a desktop or laptop computer and then print the completed form. 

Alternatively, you may print the form and then fill it out by hand. Please write legibly.
